Source File to Template File using VBA

I'm trying to create a template that can be used by multiple parties to populate a file with specific fields from a source file. The source file is placed on a shared drive contains various fields and is a status file of all open repairs in our facility. The template file only carries fields that are of value to our customer. The person using the template should have an input field (customer numbers) as soon as the file is opened and the macro populates the template file with the relevant fields from the source file.

To better illustrate your problem, you should post your workbook. It will make clear what you are referring to in your post. You should also include before and after examples of the data and comments about results you expect.

If your workbook contains sensitive or confidential information, please be sure to change the names of people, phone numbers, and businesses before posting it.

Attached is a sample source file.

I would like to give the users a separate screen where they can select a customer using a combo box. Once a customer is selected, I would only want to show the Item and Status fields.

I know that we can do this using simple auto filter, but I would like to give users a combo box interface. Pivot tables are fine.
